Refinance Document Request Form

In preparation for your refinance please fill out the form below to request both financial documents as well as the necessary 6(d) certificate. All condominium fees must be paid in full including through the proposed closing date. Otherwise Management will not be able to issue a 6(d) certificate. Please make sure that management has a minimum of (7) business days to process your 6(d). An additional fee will be issued if the 6(d) needs to be expedited.
Prior to paying your final condominium payment, please make sure that management has added the cost associated with the processing of the 6(d) on your owner ledger. You will be able to make that payment as you typically do through your owner portal via (ACH or CC). If you want to pay using a check, please send to: Boston Green Management, LLC. Some lenders require a non-standard/lender-specific questionnaire instead of the standard questionnaire included with the Unit Transfer Fee. Non-standard questionnaires are provided upon request and at an additional fee of $300 because it requires legal review prior to production. This fee is applied directly to the unit ledger at the time of the request.

By submitting this form the undersigned hereby authorizes Boston Green Property Management to release information in all matters concerning the unit and the association including pending litigation (if any). Information about this transaction is being provided by Boston Green Property Management in its capacity as managing Agent for the Association to the best of its knowledge and belief. Information is gathered from sources deemed to be reliable, however, Boston Green Property Management does not guarantee the accuracy of the information. The undersigned also agrees to pay Boston Green Property Management the total amount due for the above Documents or Services and further agrees to pay a minimum charge of $250 in collection costs should Boston Green Property Management be required to pursue collection action for payment of ordered Documents and/or Services.
